Job summary

Humana is seeking a Transition/Custodial Prevention Specialist to work with members in Illinois to develop personalized care plans, coordinate services, and promote better health outcomes and independence. You will meet members in the field at their homes, NF facilities, or a location of choice, assess goals and barriers, and connect them with LTSS, HCBS, and community resources.

The role requires travel (75–90%), a Bachelor’s degree in a related field, and experience with Medicare/Medicaid;

Qualifications

• Two (2) years of experience working with Medicare and Medicaid recipients, long-term care services, HCBS, or managed care organizations.
• One (1) or more years of professional experience working with members in custodial and transitional care.
• Experience using a variety of electronic information applications/software programs including electronic medical records.

Responsibilities

• Meet members in the field at home, NF, or location of the member's choice.
• Assess members' goals, needs, and barriers to develop plans of care.
• Coordinate LTSS, HCBS, and community resources to promote well-being and independence.
