Job summary

Qualifications

• Must reside in Illinois.
• Two years of experience with Medicare/Medicaid recipients, LTSS, HCBS, or managed care.
• One year or more in custodial and transitional care.
• Experience with electronic medical records and related software.

Responsibilities

• Meet members in the field at home, NF, or member location to assess goals and needs.
• Develop and modify Individual Service Plans with care team input.
• Coordinate LTSS, HCBS, housing, and community resources to meet member goals.
• Connect members with services and governmental payer sources and providers.
• Educate members on Medicaid eligibility and care options.
