Job summary

Humana is seeking a Transition/Custodial Prevention Specialist to work one-on-one with members to create personalized care plans, coordinate services, and empower them to achieve better health outcomes and greater independence.

You will meet members in the field at home, nursing facilities, or a location of the member's choice, assess goals and barriers, and connect with LTSS providers and community partnerships to promote well-being.

Qualifications

• Must reside in Illinois.
• Bachelor's degree in social sciences, social work, human services, or related field.
• Two (2) years of experience with Medicare/Medicaid recipients, LTSS, HCBS, or managed care.
• One (1) year or more of experience with custodial and transitional care.
• Experience with electronic information applications/software including EMR.
• Ability to travel regionally to meet members and families.

Responsibilities

• Meet members in the field at home, NF, or location of the member's choice.
• Assess goals, needs, and barriers, and develop plans of care.
• Connect members with LTSS, HCBS, and community partners.
• Collaborate with care teams to address health-related social needs.
• Support members and caregivers using an interdisciplinary approach.
