Job summary

Qualifications

• Bachelor's degree in social sciences, social work, human services or related field.
• Two (2) years of experience with Medicare/Medicaid recipients, long-term care, HCBS, or managed care.
• One (1) year or more working with custodial and transitional care.
• Experience with electronic information applications including EMR.
• Ability to travel in the region to meet members and families.

Responsibilities

• Develop and modify Individual Service Plans with care team involvement.
• Provide LTSS navigation and align services to member goals.
• Educate members on Medicaid eligibility and LTSS options.
• Coordinate with providers and community partners for HCBS.
• Engage in person-centered planning to address health-related social needs.
