Zusammenfassung

Humana in Illinois is seeking a Transition/Custodial Prevention Specialist to work one-on-one with members to create personalized care plans, coordinate services, and empower them to achieve better health outcomes and greater independence.

You will meet members in their homes, nursing facilities, or locations of the member's choice. You will assess goals, needs, and barriers, then connect members with LTSS and HCBS while collaborating with care teams to promote person-centered planning.

Qualifikationen

• Must reside in Illinois.
• Bachelor's degree in social sciences, social work, human services, or related field.
• Two years of experience with Medicare/Medicaid recipients or HCBS.
• One year of custodial and transitional care experience.
• Experience with electronic information applications including EMRs.
• Ability to travel regionally to meet members and families.

Aufgaben

• Provide LTSS support with focus on health-related social needs and psychosocial support.
• Develop plans of care and coordinate LTSS providers and community partners.
• Interact with care teams to address member goals and barriers using person-centered thinking.
• Assist navigation of LTSS, social, housing, and other services regardless of funding sources.
• Educate members to maintain Medicaid eligibility and complete assessments.
• Facilitate interaction with payer sources and interdisciplinary teams.
