Humana is seeking a Transition/Custodial Prevention Specialist to work with members in Illinois, primarily in DeKalb and surrounding counties. You will assess goals and needs, coordinate LTSS and HCBS, and connect members with community resources to improve health outcomes and independence.
In this field role, you will meet members in their homes or preferred locations, collaborate with care teams, and drive person-centered plans while navigating Medicaid and related services.
