Job summary

Rocky Mountain Human Services in Denver, CO is seeking a part-time Case Manager to support LTSS members under Medicaid and state programs. You will assess needs, coordinate services, and ensure timely PARs and plans, with visits to member residences when required.

The role emphasizes empathy, HIPAA confidentiality, and strong communication as you work with members, families, and care teams to meet service goals and secure appropriate resources.

Qualifications

• Bachelor’s degree or equivalent in LTSS field.
• 5+ years of LTSS experience or relevant background.
• Knowledge of Medicaid/state-funded programs and PARs.
• Able to navigate client management systems with HIPAA compliance.

Responsibilities

• Respond to member, family, or provider inquiries within two business days.
• Conduct in-person eligibility and monitoring visits at the member’s residence.
• Schedule assessments and service plans with the member and care team.
• Ensure PARs and plans align with assessments and service plans.
• Complete required CDASS/IHSS paperwork for annual plans.
• Monitor incidents and respond to follow-up requests promptly.
• Coordinate with providers to secure accurate PARs and plans.
• Maintain confidentiality and professional standards in all interactions.
