Qualifications

• Bachelor's degree in human behavioral sciences or 5+ years LTSS experience, or a combination thereof.
• 1+ years of experience with MS Office (Word, Excel, Outlook).
• Access to reliable transportation and ability to travel up to 50% within the territory to meet members.

Responsibilities

• Handle intake, screening, referrals, assessments, and development of care plans; ongoing case management and documentation.
• Maintain confidentiality per employee handbook and relevant policies.
• Conduct regular contact with members and update assessments and care plans in the system.
