Job summary

Montrose County in Colorado is seeking LTSS Case Manager I/II to help individuals with disabilities, older adults and vulnerable community members access services that allow them to remain safe, healthy, and independent at home and in the community.

The role involves assessments, person-centered planning, service coordination, advocacy, and documentation to meet program standards. Regular field visits and collaboration with a multidisciplinary team are essential.

Qualifications

• Bachelor’s degree in Social Work or a related field.
• Five years of LTSS-related experience or combination of education and experience.
• Experience with long-term care, gerontology, disability services, or related fields.

Responsibilities

• Build and maintain relationships with members, families, providers and interdisciplinary teams.
• Conduct screenings, assessments and ongoing evaluations to determine eligibility and needs.
• Develop and maintain person-centered support plans to promote independence and quality of life.
• Coordinate and monitor services to meet member needs per program standards.
• Advocate for member needs and help navigate available programs and resources.
• Manage an assigned caseload with timely follow-up and accurate documentation.
• Conduct in-person home, community and provider visits as required.
• Maintain compliant documentation in required systems and timelines.
• Respond to urgent or crisis situations with sound judgment and empathy.
