Job summary

UnitedHealth Group seeks a LTSS Case Manager in Colorado to provide in-home and in-facility case management, intake, and assessment. You’ll develop and monitor care plans, maintain confidentiality, and ensure timely documentation across systems.

Travel up to 75% within Lake, Chaffee, Fremont, or Custer Counties is required. Required are a bachelor’s in human behavioral sciences or related field, 5+ years LTSS experience or equivalent, and proficiency with MS Office.

Qualifications

• Bachelor’s degree in human behavioral sciences fields (human services, social work, psychology, public health)
• 5+ years of LTSS experience or a combination of education and experience
• Proficient in MS Office (Word, Excel, Outlook)

Responsibilities

• Handle intake, assessment, and development of care plans and case management.
• Maintain confidentiality and accurate documentation in the prescribed system.
• Conduct quarterly and semi-annual face-to-face contacts with members as required.
• Complete annual reassessments and update care plans accordingly.
• Monitor service delivery within the approved Support Plan and PAR.
• Participate in CMA training and agency in-service sessions.
• Travel to assigned territory and ensure timely visits.
• Adhere to CMA contract requirements and maintain quality work.
