Job summary

Molina Healthcare is hiring a Health Care Case Manager to support care management and LTSS activities. You will collaborate with a multidisciplinary team to coordinate member care across the continuum and ensure progress toward outcomes.

Responsibilities include comprehensive assessments, waiver enrollment processes, care plan development, and ongoing monitoring. Travel may be required 25–40% based on state contracts and member needs. Strong licensure and healthcare experience are essential.

Qualifications

• At least 2 years in health care with care management and LTSS exposure.
• Licenses in social work or related field required in applicable states.
• Excellent communication and organization skills.

Responsibilities

• Complete member assessments within regulatory timelines, including home visits.
• Coordinate waiver enrollment/disenrollment processes and care plans.
• Monitor and adjust care plans with care team and stakeholders.
• Promote integration of behavioral health and LTSS services.
• Assess medical necessity and authorize waiver services.
• Facilitate ICT meetings for service approvals/denials.
• Travel up to 25–40% locally as required by state contracts.
