Qualifications

• Requires 2+ years in health care with 1+ year in care management or LTSS.
• LCSW/LMSW or equivalent licensure active in state of practice.
• Bachelor's degree may be required in some states/contractual requirements.
• Must have valid driver's license, reliable transportation, and auto insurance for travel requirements.

Responsibilities

• Completes comprehensive member assessments within regulated timelines and may include in-person home visits.
• Facilitates waiver enrollment and disenrollment processes and coordinates care plans.
• Develops and implements care plans with members, caregivers, and health professionals.
• Monitors care plan effectiveness and documents interventions and goal achievement.
• Promotes integration of medical, behavioral health, LTSS, and community resources.
