Qualifications

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including home visits as required.
• Develops and implements care plans with members and care teams.
• Monitors progress and adjusts care plans to meet outcomes.
• Promotes integration of services across behavioral health and LTSS.
• Facilitates coordination meetings and documents care activities.
• Travel 25–40% as required by contract/state.
