Job summary

Molina Healthcare is seeking a Care Coordinator to support long-term services and supports (LTSS) for high-need members. You will collaborate with a multidisciplinary team to coordinate integrated member care across settings, ensuring progress toward outcomes and cost-effective care.

Responsibilities include assessments, waiver enrollment, care plan development, and ongoing monitoring. Travel up to 40% locally may be required; relevant licensure and state-specific requirements apply.

Qualifications

• At least 2 years of health care experience with LTSS preferred.
• Active driver's license and reliable transportation.
• Ability to travel 25–40% locally.
• Strong communication and documentation skills.
• Licensure or certification per state requirements as applicable.
• Experience with waiver services and interdisciplinary teams preferred.

Responsibilities

• Complete comprehensive member assessments within regulatory timelines.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and monitor care plans with members, caregivers, physicians and care team.
• Coordinate LTSS and behavioral health services for continuity of care.
• Attend ICT meetings for approval/denial of services and collaboration.
• Use motivational interviewing to educate and motivate change during member contacts.
