Qualifications

• 2+ years health care exp with LTSS and care management.
• Valid, unrestricted driver's license for job travel.
• Experience with diverse populations and settings.
• Strong verbal and written communication.
• Proficiency with Microsoft Office and online portals.
• Experience with substance use disorders in some states.

Responsibilities

• Complete comprehensive member assessments within regulated timelines.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members and care team.
• Monitor and adjust care plans based on progress.
• Promote integration of LTSS with behavioral health and community resources.
• Assess medical necessity and authorize waiver services.
• Coordinate ICT meetings for approval/denial of services.
• Use motivational interviewing and Molina guidelines.
• Identify barriers and provide support for psycho/social, financial, and medical obstacles.
• Travel 25-40% as required by state contracts.
