Molina Healthcare is seeking a Care Manager to support LTSS activities and collaborate with a multidisciplinary team to coordinate member care across the continuum. The role focuses on ensuring progress toward outcomes and contributing to cost-effective, quality care.

Responsibilities include comprehensive assessments, waiver planning, care plan development, and ongoing monitoring. Travel may be required to meet state contract requirements.

Qualifications

• At least 2 years of health care experience, with LTSS exposure.
• 1 year in care management or related medical/behavioral health setting.
• License/certification only if required by state contract; must be active if applicable.
• Valid driver’s license, reliable transportation, and insurance for travel.

Responsabilités

• Provide LTSS care management support and coordinate care across settings.
• Complete comprehensive member assessments within timelines; conduct home visits as needed.
• Develop and implement care plans with members, families, and providers.
• Monitor and adjust care plans; document outcomes and interventions.
• Promote service integration across behavioral health, LTSS, and community resources.
• Assess medical necessity and authorize waiver services; review benefits.
• Facilitate ICT meetings for service approvals; collaborate with teams.
• Identify barriers to care and assist with psycho/social/financial obstacles.
• Travel 25-40% locally as required by state contracts.
