Job summary

Molina Healthcare is seeking a Care Manager to support LTSS care coordination and collaborate with a multidisciplinary team. The role focuses on ensuring high-need members progress toward outcomes and delivering quality, cost-effective care.

The position involves developing and monitoring care plans, coordinating waiver services, and guiding members and caregivers through enrollment processes in multiple care settings. Travel is required to support field activities.

Qualifications

• At least 2 years of health care experience with LTSS exposure.
• Clinical licensure and/or certification required only if state contract warrants it.
• Bachelor’s degree may be required in some states.
• Valid and unrestricted driver’s license and reliable transportation for travel.
• Strong communication and relationship-building skills.

Responsibilities

• Completes comprehensive member assessments within regulated timelines and conducts in-person home visits as required.
• Facilitates waiver enrollment/disenrollment processes.
• Develops and implements care plans with members and care network.
• Monitors care plans, documents interventions, and suggests adjustments.
• Promotes integration of LTSS and behavioral health services.
• Assesses medical necessity and approves waiver services.
• Collaborates with licensed care managers and leadership as needed.
• Performs travel as required (25-40%).
