Qualifications

• 2+ years in health care with LTSS/care management experience.
• Licensed/certification required only if mandated by state contract.
• Bachelor's degree may be required in some states.
• Valid driver's license and reliable transportation for travel.

Responsibilities

• Completes comprehensive member assessments within regulated timelines.
• Facilitates waiver enrollment and disenrollment processes.
• Develops and implements care plans with members and care network.
• Monitors care plan effectiveness and documents outcomes.
• Promotes integration of behavioral health and LTSS services.
• Identifies barriers and coordinates care to address psycho/social needs.
