Job summary

Molina Healthcare is seeking a Care Manager to support LTSS-related activities and care coordination. You will collaborate with a multidisciplinary team to ensure quality, cost-effective member care across the continuum.

Responsibilities include member assessments, waiver enrollment, care plan development, and monitoring. Travel up to 40% may be required based on location and contract requirements. Strong communication and organizational skills are essential.

Qualifications

• At least 2 years of health care experience including LTSS exposure.
• 1 year in care management or related medical/behavioral health setting.
• Valid driver’s license and reliable transportation.
• Ability to work independently with minimal supervision.
• Proficient in navigating online portals and databases.
• Strong verbal and written communication skills.

Responsibilities

• Complete comprehensive member assessments within timelines, including in-person home visits.
• Facilitate comprehensive waiver enrollment and disenrollment processes.
• Develop and implements care plans with members and care network.
• Monitor care plans and adjust interventions as needed.
• Promote integration of services across behavioral health and LTSS.
• Assess medical necessity and authorize waiver services.
• Coordinate interdisciplinary care team meetings.
• Travel 25-40% locally as required.
