Job summary

Molina Healthcare is seeking a care coordination professional to support LTSS member care. You will complete comprehensive assessments, develop and monitor care plans, and collaborate with a multidisciplinary team to ensure high-quality, cost-effective services.

This role requires LTSS exposure, strong communication, and the ability to drive waiver enrollment, monitor plans, and coordinate across providers. Expect local travel and work across care settings to remove barriers to care.

Qualifications

• At least 2 years in health care, incl. LTSS and care management experience.
• Licensure required only if mandated by state; active in state of practice.
• Bachelor's degree in health care or related field may be required by state.
• Valid driver's license, transportation, and insurance for travel requirements.

Responsibilities

• Complete comprehensive member assessments within timelines, including home visits.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, caregivers and providers.
• Monitor care plan effectiveness and adjust as needed.
• Promote integration of behavioral health and LTSS services.
• Assess medical necessity and authorize waiver services.
• Educate on benefits and funding sources; coordinate ICT meetings.
• Identify barriers to care and address psycho/social, financial, and medical issues.
• Document interventions and track goal achievement; ensure continuity of care.
• Travel locally 25-40% as required by state/contractual needs.
