Job summary

Molina Healthcare is seeking a Care Coordinator to support care management and LTSS activities. You will collaborate with a multidisciplinary team to coordinate integrated member care across settings and help achieve desired outcomes.

The role involves developing care plans, assessing needs, and facilitating waiver services, with travel as required by state contracts. Strong communication and organizational skills are essential.

Qualifications

• At least 2 years of health care experience, LTSS, and care management.
• Licensure/certification required only if state contract mandates; active/unrestricted license if licensed.
• Bachelor's degree in a health care related field may be required per state.
• Valid driver’s license and reliable transportation for travel.
• Demonstrated knowledge of community resources.
• Ability to work independently with minimal supervision.

Responsibilities

• Completes comprehensive member assessments within timelines, including in-person home visits.
• Manages waiver enrollment/disenrollment processes.
• Develops and implements care plans with members, caregivers, and providers.
• Monitors care plans, documents interventions, and adjusts as needed.
• Promotes integration of LTSS with behavioral health and community resources.
• Assesses medical necessity and authorizes waiver services.
• Evaluates benefits and advises funding sources.
• Facilitates interdisciplinary care team meetings for approvals/denials.
• Educates and motivates members using motivational interviewing.
• Identifies barriers to care and coordinates psycho/social, financial, and medical support.
• Identifies critical incidents and develops prevention plans.
• Collaborates with licensed care managers as needed.
• Travel 25-40% may be required depending on state requirements.
