Job summary

Molina Healthcare is seeking a Care Manager to support LTSS-focused activities and coordinate member care across the continuum. You will work with a multidisciplinary team to drive quality and cost-effective outcomes for high-need members.

You will conduct comprehensive assessments, facilitate waiver processes, and implement individualized care plans while navigating barriers to care. Travel up to 40% may be required based on state contracts.

Qualifications

At least 2 years healthcare experience with LTSS, including care management or equivalent.
Valid and unrestricted driver's license with reliable transportation for travel requirements.
Strong verbal and written communication skills in diverse settings.
Experience with care management, coordination, and collaborating with multidisciplinary teams.

Responsibilities

Complete comprehensive member assessments within regulated timelines, including home visits when required.
Coordinate waiver enrollment/disenrollment processes and develop care plans with the team.
Monitor care plan effectiveness and document interventions and goal achievements.
Promote integration of LTSS, behavioral health, and community resources to ensure continuity of care.
