Job summary

Molina Healthcare is seeking a Care Coordinator to support LTSS members through comprehensive assessments, care planning, and coordination of waiver services. You will collaborate with a multidisciplinary team to ensure timely care and address barriers.

The role requires clinical licensure where mandated, travel up to 40% for state contracts, and a strong ability to navigate community resources and complex workflows.

Qualifications

• At least 2 years of health care experience, including LTSS exposure.
• Clinical licensure and/or certification required only if mandated by state contract/regulation.
• Bachelor’s degree in a health care related field may be required in some states.
• Must have valid driver's license, reliable transportation, and auto insurance for travel.
• Proven knowledge of community resources.
• Ability to work independently with minimal supervision.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitates waiver enrollment and disenrollment processes.
• Develops and implements care plans with members and care team.
• Monitors care plan effectiveness and documents interventions.
• Promotes integration of services across care settings and resources.
• Assesses medical necessity and authorizes waiver services.
• Evaluates benefits and advises on funding sources.
• Facilitates ICT meetings for approvals/denials.
• Uses motivational interviewing to educate and motivate change.
• Identifies barriers to care and provides coordination for psycho/social/financial obstacles.
• Develops prevention plans for member health and welfare.
• Collaborates with licensed care managers as needed.
• Travel 25-40% may be required.
