Job summary

Molina Healthcare seeks a Care Manager to support LTSS and care coordination across multidisciplinary teams. You will participate in comprehensive assessments, develop and monitor care plans, and facilitate waiver enrollment while addressing barriers to care. Qualifications

• 2+ years healthcare experience with LTSS or care management in relevant settings.
• Licensed or certification as required by state contracts or regulations.
• Knowledge of community resources and waiver services.

Responsibilities

• Assist with comprehensive member assessments within timelines, including home visits.
• Coordinate waiver enrollment/disenrollment and care planning with the care team.
• Monitor and adjust care plans based on outcomes and barriers.
• Promote integrated service delivery across behavioral health and LTSS.
• Support ICT meetings and document interventions and decisions.
• Navigate funding sources and authorize eligible waiver services.
• Travel 25-40% locally as required by state contracts.
