Molina Healthcare seeks a dedicated care coordination professional to support LTSS member care. You will collaborate with a multidisciplinary team, conduct assessments and implement comprehensive care plans including waiver services, while addressing barriers to care and supporting member outcomes.

The role requires travel and strong communication skills to coordinate services efficiently across settings. Experience with LTSS and waiver processes is preferred.

Qualifications

• Minimum 2 years of healthcare experience, with LTSS exposure.
• 1 year in care management or related medical/behavioral health setting.
• Valid driver’s license and reliable transportation.
• Proven ability to work independently and manage multiple tasks.
• Strong verbal and written communication skills.
• Proficiency with office software and online portals.

Responsabilités

• Support care management and care coordination for LTSS members.
• Collaborate with multidisciplinary teams to coordinate member care.
• Complete comprehensive assessments within timelines, including home visits.
• Develop and monitor care plans and waiver service plans.
• Facilitate waiver enrollment/disenrollment processes.
• Educate members on services, navigate funding sources, and remove barriers.
• Participate in ICT meetings for service decisions and documentation.
• Travel up to 25–40% depending on state requirements.
