Qualifications

Responsibilities

• Complete comprehensive member assessments within timelines, including home visits as needed.
• Facilitate waiver enrollment/disenrollment processes.
• Develop and implement care plans with members and care network.
• Monitor and adjust care plans to achieve goals and measure effectiveness.
• Coordinate with ICT for service approvals/denials.
• Perform psycho/social, financial, and medical obstacle assessments and assist members.
