Job summary

Responsibilities include assessments, waiver management, care plan development, and proactive collaboration with members and providers. Field travel will be 25-40% based on state requirements.

Qualifications

• 2+ years health care experience in LTSS/care management or related field.
• Clinical licensure/certification required only if state contract/regulation requires it.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including in-home visits.
• Facilitates waiver enrollment and disenrollment processes.
• Develops and implements care plans with members and care team.
• Monitors care plans and documents interventions and goal achievement.
• Promotes integration of LTSS with behavioral health and community resources.
• Assess medical necessity and authorize waiver services.
• Evaluate benefits and funding sources.
• Facilitate interdisciplinary care team meetings for service decisions.
• Identify barriers to care and provide care coordination support.
• Develop prevention plans for member health and welfare.
• Collaborate with licensed care managers as needed.
• Travel 25-40% may be required.
