Job summary

Molina Healthcare is hiring for a care coordination role focused on LTSS. You will support care management activities, perform comprehensive assessments, and coordinate waiver services to meet member needs across settings.

The role requires 2+ years in health care with LTSS or related fields, strong communication, and the ability to travel 25–40% locally. A valid driver’s license and state-specific licensure as applicable are needed.

Qualifications

• 2+ years in health care, LTSS experience preferred
• Care management or medical/behavioral health setting experience
• Strong communication and problem-solving abilities

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including home visits
• Facilitate waiver enrollment/disenrollment processes
• Develop and implement care plans with members, caregivers, physicians and support network
• Monitor care plans and document interventions and outcomes
• Promote integration of LTSS services and behavioral health care
• Assess medical necessity and authorize waiver services
• Collaborate with interdisciplinary care teams for approvals
• Use motivational interviewing and Molina clinical guidelines to support change
• Identify barriers to care and address psycho/social/financial obstacles
• Travel 25–40% as required by state contracts
