Qualifications

Responsibilities

• Complete comprehensive member assessments within regulated timelines.
• Develop and implement care plans with members and care networks.
• Coordinate waiver enrollment/disenrollment and monitor progress.
• Facilitate ICT meetings for service approvals/denials.
• Assess barriers to care and address psycho/social and medical obstacles.
• Travel 25-40% locally as required.
