Job summary

Molina Healthcare Inc. is seeking a Care Manager to support care coordination for LTSS across the continuum. You will collaborate with a multidisciplinary team to ensure member progress toward desired outcomes while contributing to cost-effective, high-quality care.

The role requires 2+ years in health care, LTSS or care management, a valid driver’s license, and strong communication. Travel of 25–40% may be required based on state contracts and needs.

Qualifications

• At least 2 years of health care experience, including LTSS and care management.
• Valid driver's license, reliable transportation, and license when required by state contract.
• Ability to work independently and with diverse populations.
• Proficiency with Microsoft Office and online portals.
• Experience with waiver services preferred.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including home visits.
• Facilitates waiver enrollment and disenrollment processes.
• Develops and implements care plans in collaboration with members and care team.
• Monitors care plan effectiveness and documents outcomes.
• Promotes integration of services across LTSS and behavioral health.
• Assesses medical necessity and authorizes waiver services.
• Evaluates benefits and funding sources.
• Leads ICT meetings for service approvals/denials.
• Educates and motivates members using motivational interviewing.
• Identifies barriers to care and assists with psycho/social, financial, and medical obstacles.
• Identifies critical incidents and develops prevention plans.
• Collaborates with licensed care managers as needed.
• Travel 25-40% may be required.
