Job summary

Molina Healthcare is seeking a care management professional to support LTSS activities and coordinate member care across the continuum. You will collaborate with a multidisciplinary team to achieve high-need member outcomes and minimize costs.

Responsibilities include waiver processing, care plan development, and ongoing monitoring. Willingness to travel up to 40% locally is required; strong communication and organizational skills are essential.

Qualifications

• Minimum 2 years health care experience with LTSS and care management.
• Licensed/certified only if required by state contract; active in state of practice.
• Bachelor’s degree may be required in some states.

Responsibilities

• Complete comprehensive member assessments within regulated timelines.
• Coordinate waiver enrollment/disenrollment and care plans with teams.
• Monitor care plans, document interventions, and adjust as needed.
• Promote integrated care across behavioral health and LTSS, including community resources.
• Assess medical necessity and authorize waiver services.
• Lead interdisciplinary care team meetings and coordinate with leadership.
