Job summary

Molina Healthcare, Inc. is seeking a Care Coordinator to support LTSS care management and coordination for high-need members.

You will collaborate with a multidisciplinary team to deliver integrated care across the continuum, including home visits as required, and to ensure progress toward outcomes. You will develop and monitor care plans with caregivers and health professionals, assess medical necessity, and facilitate waiver services while navigating various portals and documenting activities.

Qualifications

• At least 2 years in health care with LTSS exposure and care management experience.
• Clinical licensure or certification required only if state contract mandates it.
• Valid driver's license and reliable transportation for travel.
• Proficient with Microsoft Office and navigating online portals.

Responsibilities

• Provide care management and coordination for LTSS with high-need members.
• Complete comprehensive member assessments within regulated timelines.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with clinicians and family.
• Monitor care plan effectiveness and adjust interventions as needed.
• Promote integration of behavioral health and LTSS services.
• Assess medical necessity and authorize waiver services.
• Participate in interdisciplinary care team meetings for approvals.
• Identify barriers and coordinate psycho/social and medical support.
• Travel 25-40% regionally as required by state contracts.
