Job summary

Molina Healthcare, Inc. is seeking a care coordination professional to support LTSS activities and collaborate with a multidisciplinary team to deliver integrated member care. The role emphasizes waiver processes, care plan development, and proactive coordination across settings.

You will assess needs, authorize services, and promote access to community resources while maintaining quality and cost-effective outcomes. Travel up to 40% may be required depending on state contracts.

Qualifications

• Experience in care management or LTSS is preferred.
• Knowledge of community resources and ability to navigate portals.
• Strong verbal and written communication skills.
• Valid driver's license and ability to travel 25–40%.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, caregivers, physicians and support network.
• Monitor care plan effectiveness and adjust interventions as needed.
• Promote integrated LTSS and behavioral health services for continuity of care.
• Assess medical necessity and authorize waiver services.
• Coordinate ICT meetings for approval/denial of services.
• Travel 25–40% as required by state contract.
