You will collaborate with a multidisciplinary team to develop and monitor care plans, assist with waiver services, and ensure timely assessments and coordination for high-need members. Responsibilities include facilitating waiver enrollment, monitoring outcomes, and addressing barriers to care while promoting integrated services.

Qualifikationen

• 2+ years healthcare experience with LTSS or disability/chronic conditions.
• 1 year in care management or related medical/behavioral health setting.
• Valid driver's license and reliable transportation.
• Ability to travel 25–40% locally.
• Proficient in Microsoft Office and online portals.

Aufgaben

• Completes comprehensive member assessments within regulated timelines, including in-person home visits as required.
• Facilitates comprehensive waiver enrollment and disenrollment processes.
• Develops and implements care plans with members, caregivers, physicians and/or other professionals.
• Monitors care plan effectiveness and documents interventions and outcomes.
• Promotes integration of services for LTSS and behavioral health care.
• Assesses medical necessity and authorizes waiver services.
• Evaluates benefits and funding sources for members.
• Facilitates ICT meetings for approval/denial of services.
• Uses motivational interviewing and Molina guidelines to educate and support change.
• Identifies barriers to care and provides coordination to address psycho/social and medical obstacles.
• Develops prevention plans for member health and welfare.
• Collaborates with licensed care managers/leadership as needed.
• 25-40% local travel may be required.
