Responsibilities

• Complete comprehensive member assessments within timelines.
• Coordinate waiver enrollment/disenrollment processes.
• Develop and monitor care plans with care network.
• Support integration of behavioral health and LTSS services.
• Identify barriers to care and address psycho/social obstacles.
