Job summary

Molina Healthcare seeks a dedicated Care Manager to support LTSS care coordination for high-need members in the United States. You will collaborate with a multidisciplinary team to ensure integrated, quality, cost-effective member care across the continuum.

This role involves comprehensive assessments, waiver enrollment, care plan development, monitoring outcomes, and facilitating ICT meetings, using motivational interviewing and Molina clinical guideposts to drive positive changes.

Qualifications

• 2+ years healthcare experience, including LTSS.
• Experience with disability/chronic conditions LTSS and care management.
• Willingness to travel 25-40% as required by state/contract.
• Valid driver’s license, transportation and insurance for job travel.
• Strong verbal and written communication, documentation and coordination.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including home visits.
• Facilitate waiver enrollment/disenrollment processes.
• Develop and implement care plans and waiver service plans with the member and care network.
• Monitor care plan effectiveness and adjust as needed.
• Promote integration of behavioral health with LTSS and community resources.
