Molina Healthcare is seeking a qualified Care Manager to coordinate LTSS for members with high-need potential. You will collaborate with an interdisciplinary team to ensure timely assessments, comprehensive care plans, and effective service delivery across the continuum.

The role emphasizes proactive care coordination, resource navigation, and documenting outcomes while adhering to regulatory requirements and organizational guidelines.

Qualifications

• 2+ years in health care with care management or LTSS exposure.
• Active licensure in state (LCSW/LMSW/APSW/CHES/LPC/LPCC/LMFT/PhD/PsyD) or equivalent.
• Bachelor’s degree in health care field may be required by state contracts.
• Valid driver’s license and reliable transportation for travel.
• Strong independent work style with excellent time management.

Responsibilities

• Complete comprehensive member assessments within required timelines, including home visits where needed.
• Manage waiver enrollment/disenrollment processes.
• Develop and implement member care plans with healthcare professionals and support networks.
• Monitor care plans and document progress toward goals and adjust as needed.
• Promote integration of behavioral health and LTSS with community resources.
