Job summary

Molina Healthcare, Inc. is seeking a Care Coordinator for LTSS to support care management and coordination across the continuum for high-need members.

You will collaborate with a multidisciplinary team, conduct home visits when needed, and help enroll or disenroll waiver services. You will develop and monitor care plans with members, caregivers, and health professionals, ensuring progress toward outcomes while integrating behavioral health and community resources.

Qualifications

• Minimum 2 years health care experience, including LTSS and care management.
• Licensure/certification required only if state contract or regulations demand it.
• Bachelor’s degree may be required in some states.
• Valid and unrestricted driver’s license and reliable transportation.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitates waiver enrollment and disenrollment processes.
• Develops and implements care plans with members, caregivers, physicians and care network.
• Monitors care plan effectiveness and documents interventions and goals.
• Promotes integration of services across behavioral health and LTSS.
• Assesses medical necessity and authorizes waiver services.
• Evaluates benefits and funding sources.
• Facilitates ICT meetings for service approval/denial.
• Uses motivational interviewing to educate and motivate members.
• Assesses barriers to care and provides coordination and assistance.
• Identifies critical incidents and develops prevention plans.
• Collaborates with licensed care managers as needed.
• 25–40% local travel may be required.
