Job summary

Molina Healthcare is seeking a care management professional to support LTSS activities and coordinate member care across the continuum. You will work with a multidisciplinary team to develop and monitor care plans, ensuring progress toward outcomes and cost-effective, high-quality care.

Responsibilities include comprehensive assessments, waiver processing, and collaboration with physicians, caregivers, and care teams. Travel will be required to meet state contracts and member needs.

Qualifications

• 2+ years of health care experience with LTSS or disability/chronic conditions.
• Certification/licensure required only if state contract mandates, active/unrestricted in state of practice.
• Bachelor's degree in a health care related field may be required in some states.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, caregivers, physicians, and network.
• Monitor care plan effectiveness and document interventions and goals.
• Promote integration of behavioral health and LTSS for continuity of care.
• Assess medical necessity and authorize waiver services.
• Collaborate with interdisciplinary care team and leadership as needed.
• Travel 25-40% based on state/contract requirements.
