Qualifications

• 2+ years in health care, including care management or managed care.
• 1+ year working with persons with disabilities, chronic conditions, LTSS.
• Active and unrestricted professional license in state of practice.

Responsibilities

• Complete comprehensive member assessments within regulated timelines.
• Develop and implement care plans with the member and care network.
• Monitor care plan effectiveness and adjust as needed.
• Coordinate LTSS and waiver services with an interdisciplinary team.
• Assess medical necessity and authorize waiver services.
• Support members to overcome psycho/social, financial, and medical barriers.
