Qualifications

• At least 2 years of health care experience with 1 year in care management or LTSS.
• Active licensure in one of the listed social work or counseling professions.
• Valid driver's license and reliable transportation for local travel.

Responsibilities

• Complete member assessments within timelines and coordinate home visits when required.
• Develop and implement care plans with members, caregivers, and providers.
• Monitor and adjust care plans based on progress and changing needs.
• Promote LTSS integration and connect members to community resources.
• Facilitate ICT meetings for service approvals and coordination.
• Travel locally 25-40% as required by state contracts.
