Job summary

Qualifications

• At least 2 years of healthcare experience including care management and LTSS.
• Active license as LCSW/LMSW/APSW or equivalent per state requirements.
• Valid driver's license and ability to travel as needed.
• Ability to work independently and manage multiple priorities.

Responsibilities

• Complete comprehensive member assessments within regulatory timelines, including home visits when required.
• Facilitate waiver enrollment/disenrollment processes and develop care plans with members and care teams.
• Monitor care plan effectiveness and adjust as needed to achieve goals.
• Promote integration of behavioral health and LTSS with community resources to ensure continuity of care.
• Identify barriers to care and coordinate with teams to address psycho/social/financial obstacles.
