Platte Health Center Avera is seeking a qualified Nurse leader to join our team as Assistant Director of Nursing. You will coordinate MDS 3.0 processes, oversee patient admissions, and provide resident care planning and supervision for a skilled care setting in Platte, SD.

The role emphasizes leadership in nursing, regulatory compliance, and collaboration with the DON and care teams to ensure high-quality, safe resident care across shifts, including potential weekends and evenings.

Qualifications

• RN license in South Dakota.
• Clinical and supervisory experience in LTC preferred.
• Ability to respond to patient status and data reflecting growth and development.
• Excellent communication and organizational skills.

Responsibilities

• Assist Director of Nursing with daily department management.
• Supervise CNAs and nurses; support DON.
• Facilitate Care Committee and Patient Safety/Risk Management meetings.
• Manage admissions for new residents; participate in rounds.
• Coordinate MDS 3.0 process and transmission; schedule.
• Prepare and update resident care plans; attend care conferences.
• Ensure compliance with MDS 3.0 and safety protocols.
• Coordinate immunizations and infection control activities.
