Job summary

Platte Health Center Avera is seeking a qualified nurse to join our team. This PRN role involves supervising and providing care to residents, using the nursing process to develop individualized plans and ensure patient safety.

SD-licensed LPN or RN with BLS is required or eligible. Prior long-term care experience is helpful but not required; actual hourly rate depends on experience.

Qualifications

• BLS certification within 6 months of hire.
• Experience in long-term care is helpful but not required.
• Ability to supervise and collaborate with care staff.

Responsibilities

Actively uses the nursing process in resident centered patient care, demonstrating accountability as an individual and team member. Synthesizes information from a variety of sources to develop and implement an individualized, age appropriate, collaborative plan of care. Provides timely, continuous, age appropriate, collaborative assessment and preparation of patients and family. Assesses patients’ and families’ age appropriate learning needs, coordinates necessary teaching and evaluates teaching outcomes. Demonstrates a knowledge of and utilizes safe and appropriate patient care protocol as set forth in the standards of practice and the unit/department policy and procedure manual relating to the needs of adult and geriatric patients. Documents in the medical record. Completes physician rounds as needed. Performs procedures with minimal supervision. Adheres to policies and procedures in the department manual. Education and/or Experience/Licensure/CertificationEducation: Eligible for or currently licensed as an LPN or an RN in the state of South Dakota. Basic Life Support is mandatory within six months of hire.Experience: Previous experience in Long Term Care is helpful, but not required.PayActual hourly rate dependent upon experience.
