Job summary

Avera Platte Health Center is seeking a qualified Patient Care Coordinator to join Platte Care Center. You will support daily nursing management, supervise CNAs and RNs, and lead care and safety committees to ensure high-quality resident care.

You will coordinate admissions, conduct daily rounds, and manage MDS 3.0 scheduling and care plans, while upholding infection prevention standards and patient rights. Strong leadership and communication are essential.

Qualifications

• Currently licensed as an RN in South Dakota.
• Clinical and supervisory experience in LTC preferred.
• Excellent communication and organizational skills.
• Ability to respond to issues reflecting patient status.
• Ability to lift up to 50 pounds and perform physical tasks.

Responsibilities

• Assists Director of Nursing with daily management of the Department.
• Supervises CNAs and Nurses.
• Directs Care Committee and Patient Safety/Risk Management Committee meetings.
• Provides input for employee evaluations.
• Coordinates admissions for new residents and performs daily resident rounds.
• Manages MDS 3.0 scheduling and care plan updates.
• Ensures timely MDS 3.0 transmission and safety checks.
