Job summary

Western Reserve Area Agency on Aging (WRAAA) is seeking an LSW Case Manager to support long-term clients age 60+ enrolled in MyCare Ohio CareSource waiver programs through assessment, care coordination, advocacy, and ongoing case management. The role blends healthcare leadership with community impact in a hybrid field-based setting.

You will conduct comprehensive assessments, develop care plans, coordinate home and community services, advocate for clients, and collaborate with interdisciplinary

Qualifications

• Current licensure as a Licensed Social Worker (LSW) through the State of Ohio Counselor and Social Work Board.
• Minimum of one year of experience working with individuals with physical or behavioral disabilities in home healthcare, medical social work, or behavioral health environments.
• Excellent communication, interviewing, and assessment skills.
• Knowledge of community resources available to older adults and individuals with disabilities.
