Western Reserve Area Agency on Aging (WRAAA) is a private nonprofit corporation designated by the State of Ohio as the planning, coordinating, and administrative agency for federal and state aging programs throughout Cuyahoga, Geauga, Lake, Lorain, and Medina Counties.
As an LSW Case Manager, you will support long-term clients age 60 and older enrolled in the MyCare Ohio CareSource waiver programs through clinical assessment, care coordination, advocacy, and ongoing case management.
