Job summary

Western Reserve Area Agency on Aging is seeking an RN Case Manager to support clients aged 60+ enrolled in MyCare Ohio CareSource waivers. This hybrid, field-based role combines clinical assessment, care coordination, advocacy, and ongoing case management to improve outcomes for older adults and people with disabilities.

Qualifications

• Current RN license through the State of Ohio Counselor and Social Work Board.
• Minimum of one year experience with individuals with physical or behavioral disabilities in home healthcare, medical social work, or behavioral health environments.
• Excellent communication, interviewing, and assessment skills.
• Knowledge of community resources available to older adults and individuals with disabilities.
