Job summary

Western Reserve Area Agency on Aging is seeking a licensed Practical Nurse to serve as Waiver Service Coordinator. The role requires coordinating home- and community-based waiver services and working with an interdisciplinary team to support independent living for older adults and individuals with disabilities.

Qualifications

• Current and unrestricted Licensed Practical Nurse (LPN) license in Ohio.
• Graduate of an accredited Practical Nursing program.
• Experience with older adults and/or disabilities is preferred.

Responsibilities

• Coordinate and monitor authorized home- and community-based waiver services.
• Conduct member contacts and home visits as required.
• Assist with implementing and monitoring waiver service plans.
• Collaborate with Care Managers and the interdisciplinary team.
• Communicate changes in health or service needs to clinical staff.

Western Reserve Area Agency on Aging (WRAAA) is a private non-profit corporation, organized and designated by the State of Ohio to be the planning, coordinating and administrative agency for federal and state aging programs in Cuyahoga, Geauga, Lake, Lorain and Medina Counties. Position Summary

The LPN Waiver Service Coordinator works collaboratively with members, caregivers, providers, Care Managers, and interdisciplinary team members to coordinate and monitor home- and community-based waiver services.

Job Duties

• Coordinate and monitor authorized home- and community-based waiver services.
• Conduct required member contacts and home and community visits.
• Assist with implementing and monitoring individualized waiver service plans.
• Coordinate services with members, caregivers, providers, Care Managers, and interdisciplinary team members.
• Identify changes in a member's health, functional status, living environment, or service needs and communicate concerns to appropriate clinical or care-management staff.
• Assist members with accessing providers, community resources, durable medical equipment, home modifications, and other authorized services.
• Monitor service delivery and follow up on service gaps, delays, interruptions, and member concerns.
• Provide members and caregivers with education and information regarding available services and community resources.
• Maintain timely and accurate documentation of member contacts, services, referrals, and follow-up activities.
• Participate in care transitions and interdisciplinary service coordination.
• Advocate for members and support their ability to live safely and independently in the community.
• Perform responsibilities within the authorized scope of practice of an Ohio Licensed Practical Nurse.
• Travel throughout WRAAA's service area as required.

Qualifications

• Current and unrestricted Licensed Practical Nurse (LPN) license in the State of Ohio.
• Graduate of an accredited Practical Nursing program.
• Previous nursing experience preferred.
• Experience working with older adults and/or individuals with disabilities preferred.
• Experience in home health, managed care, long-term care, community health, Medicaid waiver programs, MyCare Ohio, HCBS, care coordination, or service coordination is a plus.
• Compassionate, person-centered approach with strong communication and interpersonal skills.
• Strong organizational, documentation, time-management, and follow-up skills.
• Ability to work independently and effectively as part of an interdisciplinary team.
• Ability to recognize changes in member condition and appropriately communicate and elevate concerns.
• Proficiency with electronic documentation systems and Microsoft Office.
• Valid driver's license and reliable transportation.
• Ability to travel throughout WRAAA's service area, including Cuyahoga, Geauga, Lake, Lorain, and Medina Counties.
