Job summary

Western Reserve Area Agency on Aging (WRAAA) seeks an experienced LPN Patient Navigator for MyCare Ohio CareSource. This role delivers high-quality in-home visits, collects data, and collaborates with Waiver Service Coordinators to support members.

The position involves monitoring home modifications, handling emergency outreach, and assisting with service-plan reconciliation and referrals. Qualified candidates hold an active Ohio LPN license, have at least one year in home- or health-related

Qualifications

  • Current licensure as an LPN with the Ohio Board of Nursing.
  • At least 1 year of experience in home health, medical, or behavioral health.
  • Must have a valid Ohio driver’s license and auto liability insurance.

Responsibilities

  • Visit members at home or community locations to meet contractual obligations.
  • Collect data for HRA and quarterly visits; document per requirements.
  • Act as liaison with Waiver Service Coordinator staff and providers.
  • Monitor progress of home modifications and obtain satisfaction via home visits.
  • Provide in-office telephone coverage and triage with Waiver Service Coordinator Supervisor.

Western Reserve Area Agency on Aging (WRAAA) is a private non-profit corporation located, organized, and designated by the State of Ohio to be the planning, coordinating and administrative agency for federal and state aging programs in Cuyahoga, Geauga, Lake, Lorain, and Medina Counties. It is one of twelve Area Agencies on Aging (AAAs) in the state organized together with local service provider organizations and the Ohio Department of Aging (ODA) to form the state's public aging services network. Position Summary:

The LPN Patient Navigator for MyCare Ohio CareSource is a highly motivated and compassionate professional responsible for providing superior service to members through in-home visits, data collection, and collaboration with Waiver Service Coordinators. This role involves monitoring home modifications, conducting emergency outreach, claims review, and service plan reconciliation, as well as facilitating referrals for requested services.

Job Duties:
  • Performs member visits at home or other community locations to meet contractual obligations of Care Management Contact Schedules.
  • Data gathering of HRA and quarterly visits; collaborate with WSC and document per requirements.
  • Acts as a liaison with Waiver Service Coordinator staff and providers.
  • Monitors the progress of home modifications through completion and obtaining satisfaction with modification via a home visit.
  • Provides in-office telephone coverage and triaging with Waiver Service Coordinator Supervisor regarding the disposition of calls.
  • Assists with completing Emergency Response Outreach to members at the request of the MCO during weather events or other emergency situations.
  • Completes claims review of members services, including follow up with members and providers when discovery is made of any discrepancy between services authorized and delivered.
  • Completes reconciliation of members’ Service Plans, including entering of Care Manager authorized services.
  • Completes referrals to providers of services requested by member.
  • Management of various spreadsheets or reports required by the MCO or internal management staff.
  • Assists in the review of electronic files per the direction of supervisor.
  • All staff have the responsibility and authority to propose new ways to improve quality and to fully participate in quality improvement efforts.
  • Performs other duties as assigned.
Qualifications:
  • Current licensure as a License Practical Nurse with the Ohio Board of Nursing.
  • At least one year of experience working with individuals with physical or behavioral health disabilities in a home health care, medical, or behavioral health environment is required.
  • Must have a current valid Ohio driver’s license, proof of auto liability insurance.
  • Must be able to travel in own personal vehicle within the 5-county WRAAA region during work hours.
  • Ability to accurately and timely document in members’ electronic files.
  • Knowledge of use of general office equipment and computer skills.
  • Proficiency in interviewing skills, listening, and excellent interpersonal skills.
  • Excellent written communication skills with the ability to complete documentation in grammatically correct format.
  • Strong organization skills with the ability to prioritize.
  • Ability to work independently within the team structure.
  • Flexibility to adjust to variations in the work environment.
  • Ability to work with multi-cultural members and experience with an interpreter.
  • Knowledge of medical terminology and basic disease processes.
  • Comfortable in going into members’ homes, hospitals, and nursing facilities.
  • Must be detail oriented, have excellent interpersonal communication skills and the ability to work effectively with others.
